At VERVE Lash & Skin Lounge, we truly value the time of every client. Our policies help us keep appointments running smoothly so we can give everyone the best experience possible to each client.

VERVE Skin Intake Forms

When an appointment is booked with VERVE Skin, an intake form will be sent and must be completed 24–48 hours before the scheduled service.

This form is essential to identify any possible contraindications to your treatment. It provides important details about any allergies, current skincare products, medications, and health considerations, which allows VERVE Skin to design the safest and most effective treatment plan.

If the intake form is not completed within the required timeframe:

  • A $25 fee may be applied for rescheduling due to lack of completed information.

  • If the client arrives without a completed form and the service cannot proceed (due to missing or necessary information), the appointment will be treated as a no-show and charged 100% of the service price.

Completing this form on time ensures VERVE Skin can prepare properly and provide a safe, customized, and effective treatment experience.

Credit Card Payments

When using a credit card for payment, please note there is a 3% processing fee added to the total. This fee is charged by the payment processor and is non-refundable. To avoid this fee, you are welcome to pay with cash, debit card, or other payment methods.

Rescheduling

Once an appointment has been rescheduled twice, a 50% non-refundable deposit will be required if a third rescheduling needs to be made.

Cancellations

We kindly ask that any cancellations or reschedules be made at least 24 hours in advance.

Cancellations made with less than 24 hours’ notice may be subject to a 50% service charge.

No-Shows

If an appointment is missed without notice, the scheduled service fee will be charged in full (100% of the service price).

Late Arrivals

If you arrive more than 15 minutes late, we may need to reschedule your appointment to ensure the best results. In this case, a 30% service fee will apply.

Feeling Unwell

Your health and comfort matter to us. If you’re feeling under the weather or experiencing any symptoms of illness, please let us know as soon as possible so we can reschedule your appointment. This helps protect the wellness of all our clients and staff.
